In a medium-sized city, the municipal government
decided to implement a new health management
strategy, following the principles of decentralization as
established by the Basic Operational Standard of the
Ministry of Health. The process involved extensive
restructuring, including the transfer of responsibilities to
levels closer to the population. The redefinition of roles
resulted in greater autonomy for local health units, which
could now adapt services to the specific needs of the
community. Moreover, new relationships were
established between municipal, state, and federal
spheres, with the creation of health councils to ensure
social control and active community participation in health
management. Considering the fundamentals of
decentralizing the management of health actions and
services, according to the Basic Operational Standard of
the Ministry of Health, and reflecting on the practical
